The 2012 Application Form is available to the left for download.
If you choose the Excel version you may download the form then complete it on your own computer for printing and mailing to the Club. You will only be able to enter information on certain lines in this Excel sheet; all other lines are blocked.
If you choose the PDF version you may download the form, print it first then complete the printed version for mailing to the Club.
Membership in the Club automatically includes membership with Tennis B.C. You should not pay a separate Tennis B.C. membership if you pay our Membership dues.
